Ridgeway Post Acute, a 78-bed skilled nursing facility in Petaluma, is seeking a dedicated and experienced Social Services Director to join our team. This role is vital in ensuring residents receive the emotional, social, and discharge planning support they need while maintaining compliance with state and federal regulations.

Key Responsibilities:

• Oversee the social services department, ensuring residents' psychosocial needs are met
• Assist residents and families with discharge planning, community resources, and care transitions
• Advocate for residents by coordinating services and support with healthcare teams
• Ensure compliance with state and federal regulations, including documentation requirements
• Work closely with the interdisciplinary team to enhance quality of life for residents
• Provide emotional and social support to residents and their families
• Address concerns, mediate conflicts, and help resolve issues related to resident care and rights

What We're Looking For:

• Experience in social services within a skilled nursing facility or healthcare setting
• Knowledge of state and federal regulations related to long-term care
• Strong leadership and problem-solving skills
