GENERAL RESPONSIBILITIES & ESSENTIAL JOB FUNCTIONS:
Coordinate with team and community services as needed to ensure that both current diseases, as well as preventative and health promotion needs are addressed as part of the holistic, whole-person, culturally-sensitive health home approach
Serve as Principle Behavioral Health Provider/Liaison for Veterans and Veteran family in Compass services
Familiarity with Veterans Organizations for referral and Care Coordination
Work with Compass Peer Support staff on ensuring we meet the needs of those in service
Identify and engage people in services in benefits of care coordination
Be the system navigator and point of contact for people in service and families and a point of direct access for asking questions and raising concerns
Navigate people toward social service benefits for which they qualify due to health status
Assist people in service in scheduling needed appointments and encouraging active participation in all aspects of their healthcare needs
MINIMUM QUALIFICATIONS:
Age 21+ years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience. Experience with Case Management or Targeted Case Management preferred
Excellent organizational and time management skills
Excellent communication skills for engagement with people in services and our community partners
High degree of professionalism and philosophy of treating the whole person is necessary
Preference given to someone who has served in the Military or is family member or Veteran for the Veteran aspect of this position
